What Makes All-on-4 Different

Most full-arch implant protocols use six or more implants per arch placed straight up and down. The All-on-4 concept changed that approach: by tilting the back two implants toward the front of the mouth, the implant tips reach into denser anterior bone while the heads of the implants emerge in the back of the arch. The angled implants act as the rear support pillars for the bridge.

The result is a fixed full-arch bridge supported by four implants instead of six or eight, with several practical advantages:

  • Sinus lifts and bone grafting can often be avoided in the upper arch
  • Surgery is typically completed in a single appointment
  • A fixed provisional bridge can usually be delivered the same day
  • Total treatment time and cost are reduced compared to multi-stage protocols

Same-Day Teeth: How That Actually Works

"Teeth in a Day" is a real outcome of the All-on-4 protocol when bone allows immediate loading. After implant placement, a fixed acrylic provisional bridge is attached to the four implants the same afternoon. You leave the office with a full arch of teeth — not a denture, not adhesive — that's screwed in.

The provisional functions like teeth from the first night. We restrict harder foods (raw vegetables, nuts, hard meats) during the three to six months of healing while the implants integrate with your bone. After integration is confirmed, the provisional is replaced with a definitive final bridge in stronger materials.

Who's a Good All-on-4 Candidate

All-on-4 works well for patients who:

  • Are missing most or all teeth in an arch, or have remaining teeth that need to be removed
  • Want fixed (non-removable) teeth
  • Have adequate bone in the front of the arch to support angled posterior implants
  • Want a single-stage, efficient treatment timeline
  • Are healthy enough for routine implant surgery

All-on-4 may not be the best protocol if:

  • Bite forces are unusually heavy (e.g., severe bruxism) — we may recommend more implants
  • Bone is severely compromised throughout the arch — alternative protocols may apply
  • The patient prefers separate, individual implant restorations for back teeth — different treatment plan

The number of implants in a full-arch case isn't a marketing decision. It's a clinical one based on bone, bite, and longevity goals. We don't add implants to inflate the bill, and we don't subtract them to undercut a quote.

Will I leave with teeth the day of surgery?

Yes, in most cases. The provisional bridge is attached the same day. You won't go home toothless or in a denture.

How long do All-on-4 implants last?

The implants themselves can last decades. The provisional bridge is replaced with a final at three to six months. Final bridges typically last 15-25 years.

Is All-on-4 cheaper than All-on-6?

Usually somewhat — fewer implants means lower implant material cost. But the final cost depends on the prosthesis materials and whether grafting or extractions are needed.

Can I do All-on-4 on just my upper arch?

Yes, single-arch All-on-4 cases are common. Many patients have natural teeth or partial dentures opposing the arch being restored.

What if an implant fails?

Implant failure rates are low (under 5% with appropriate planning) but not zero. If an implant fails, we replace it. With four implants, the bridge generally remains stable while replacement is healing.

Can I use Tricare or insurance for All-on-4?
